Third Party Notification is used for the convenience of a person who would like for a third party to be notified when his/her account has become past due and may be subject for disconnection.
This agreement is for notification purposes only and in no way obligates the third party to be financially responsible for any utility charges, fees or penalties, nor shall this agreement prevent or delay termination of utility services. The agreement will remain in effect until termination has been requested in writing or in person by either the utility customer or the third party.
Public Utilities Code Section 10010.1(c):
Every public utility shall make available to its residential customers who are 65 years of age or older, or who are dependent adults as defined in paragraph (1) of subdivision (b) of Section 15610 of the Welfare and Institutions Code, a third-party notification service, whereby the public utility will attempt to notify a person designated by the customer to receive notification when the customer's account is past due and subject to termination. The notification shall include information on what is required to prevent termination of service. The residential customer shall request for third-party notification on a form provided by the public utility, and shall include the written consent of the designated third party. The third-party notification does not obligate the third party to pay the overdue charges, nor shall it prevent or delay termination of service.
Customer's who request to have a third party authorized on their account must complete the appropriate form. Once this complete form is received by the Utility Customer Services office a consent form will be mailed to the third party designee on your behalf.
Third Party Notification will be established when the Utility Customer Services office has received the completed consent form.
Complete the following steps to request Third Party Notification:
- Print the Third Party Notification Request form*.
- Complete and sign the Third Party Notification form.
- Return the completed form with your next utility payment or mail the form to:
